Our team includes:

Hospice Physician: All patients under Comfort Hospice & Palliative Care treatment are under close monitoring by our hospice physician. The physician keeps a close watch on the patient’s illness and prescribes appropriate medication when necessary. In addition, the physician works closely with other team members, such as our nurses, but also with the patient’s preferred physician, in order to properly manage symptoms and improve quality of life.

Hospice Nurse: Our nurses specialty in evaluating and treating patient’s pain and symptoms. In addition to hands on care, they work closely with the family members in educating and guiding them for treatment and care practices.

Hospice Aide: Our hospice aides are certified nursing assistants who assist patient’s with personal care related tasks, such as bathing, oral care, and dressing. They help take the load off of family caregivers by performing housekeeping duties.

Hospice Social Worker: Social workers provide emotional support to both the patient and family members. In addition to counseling and support, they help organize and arrange all logistics of the patient’s treatment, such as coordinating with insurance companies, assisting with finances, funeral planning, among many other tasks. Our social workers will be available to assist with many tasks whenever needed.

Hospice Volunteer: Our volunteers are passionate about improving quality of life and uplifting patients and their families. They specialize in hospice related companionship, and at times facilitate care. Volunteers play a crucial role in hospice by visiting patients and crafting delightful surprises which creates memorable experiences for the patients and families.

Hospice Chaplain: Our chaplains are available to assist with spiritual needs of the patient and their family. Our chaplains support all cultural and religious beliefs, and can be requested at anytime through the hospice care process.

Bereavement Specialist: Our bereavement specialist help in anticipatory grief and loss after death. Family members are eligible to receive up to 13 months of bereavement support after death, which includes direct, one-on-one support, education, and support groups. Hospice is always there to give a helping hand to those who’ve recently experienced the loss of a loved one.
