Transitional Care involved when a patient/client leaves one care setting hospital, nursing home, assisted living facility SNF, primary, physician home health and moves to another.
Nursing play a key role in promoting successful transition by developing and evaluated the transition plan and identifying and the communication barriers to the plan. Nurses must engage patient and caregiver at active partners and advocates for their healthcare and the most support needs.
Transitional Care is a temporary free of charge services to help family member to adjust and understand patient’s care.
